Simplify handling of v16i8 shuffles and fix a missed optimization.

Rename pmap_collect() to pmap_pv_reclaim() and rewrite it such that it no
longer uses the active and inactive paging queues.  Instead, the pmap now
maintains an LRU-ordered list of pv entry pages, and pmap_pv_reclaim() uses
this list to select pv entries for reclamation.

Note: The old pmap_collect() tried to avoid reclaiming mappings for pages
that have either a hold_count or a busy field that is non-zero.  However,
this isn't necessary for correctness, and the locking in pmap_collect() was
insufficient to guarantee that such mappings weren't reclaimed.  The new
pmap_pv_reclaim() doesn't even try.

Centralize the handling of the "attribute declaration must precede definition"
warning. This also makes us warn on tags, which, ironically, is the only case
gcc warns on.

    hammer2 - hardlink stabilization (3), data and inode count propagation.
    
    * Files with cached chains have to be flushed before they can be copied
      to the hardlink target, because the original inode will become a
      OBJTYPE_HARDLINK pointer which isn't allowed to have any sub-chains
      under the inode.
    
    * We also need to flush for the upcoming snapshot function to work properly
      or dirty in-memory data will not show up in the snapshot.
    
    * Propagate the inode and byte use count up the chain.  Tie the inode count
      into df's inode count (per-PFS).  The byte count and quota fields are not
      yet tied in.
    
    * Adjust stat[v]fs() to return filesystem space useage using the allocation
      iterator for now, to aid debugging.
    
    * Adjust the allocation iterator to skip reserved areas at the beginning of
      each 2GB storage zone.

Teach two-address pass to update the "source" map so it doesn't perform a
non-profitable commute using outdated info. The test case would still fail
because of poor pre-RA schedule. That will be fixed by MI scheduler.

For "process attach", make the success criterion as the inferior changes its state to 
eStateStopped.
Otherwise, mark it as a failure and say so.

 - Fail when the utility is not invoked as rtprio nor idprio.
 - use warnx() to tell the user whether a process is running in normal,
   idle or realtime priority. with the old code it would have been possible
   for another process to send data to stdout between
        printf("%s: ", p);
   and
        printf("* priority\n");
   and thus break the formatting.
 - 'rtprio 10 -0' triggeres non-intuitive behavior.  It would first set the
   priority of itself to 10 *and* would then try to execute '-0'. Of course,
   setting the priority of [id|rt]prio itself doesn't make a lot of sense,
   but it is intuitive compared to the previous behavior.
 - 'rtprio -t --1' will actually pass over the '-1' to rtprio().  Now
   invoking rtprio like this will catch the wrong usage before passing
   over the invalid argument to rtprio().
 - Garrett Cooper suggested to add further diagnostics where the failure
   occures, if execvp fails.
